New Patients


If you are joining us for the first time:

Complete Forms

Complete the new patient forms before your visit.

Bring Documents

Bring your insurance card, photo ID, and a list of current medications.

Arrive Early

Plan to arrive 15 minutes early so we can set up your chart.

Medi-share or Christian Ministry- they are not an insurance company. The patient will need to pay 2/3’s of the charges and we will file the claim.  Medi-share or Christian Ministry may use PHCS/Multiplan to process their claims, but again they are not an insurance company.

Existing Patients


If you are already established with our practice:

Log in to the patient portal for forms, test results, and secure messages.

Refill Requests

Request prescription refills through the portal or by phone.

Urgent Concerns

Contact our office directly with urgent concerns.

Prescription Refills


To manage prescriptions accurately and efficiently, please bring your prescription bottles or a current medication list (including refill counts) to each appointment. We will provide enough refills for chronic medications until your next scheduled visit.

Acute medications such as antibiotics or pain medicine usually require an office evaluation.

If you run out of refills before your next appointment:

Patient Portal

Use the Patient Portal or the Healow App.

Billing

If you do not have internet access, ask your pharmacy to send us an electronic request.

Appointments


To schedule an appointment, call (828) 294-4100 during office hours. Our staff will work with you to find a convenient time and connect you with the right provider.

If you need to cancel or reschedule, please call at least 24 hours in advance so we can offer the time to another patient in need. Same-day appointments may be available for urgent concerns, but we encourage scheduling routine visits well in advance.
